Responsibilities:

• Provide emotional and practical support to clients.
• Assist clients with daily living activities.
• Maintain accurate records of client interactions and progress.
• Develop and implement individualized care plans for clients.
• Attend regular team meetings to discuss client progress and needs.
• Provide advice and support to clients in crisis situations.
• Liaise with other professionals, such as doctors and social workers, to ensure the best possible care for clients.

• Monday to Friday
