Coordinator Community Home – Full Time

Education/Training/Certifications

  • Graduate of DSW, SSW, BST Program or other Human Services degree/diploma
  • Current CPR & First Aid and CPI Certificates
  • Applied Medications Certificate or completed training deemed equivalent by the employer
  • Fully Vaccinated – COVID-19, in accordance with Health Canada
  • Valid Full G Drivers Licence & access to a reliable vehicle
  • Demonstrated experience related to the job description
  • Ability to understand, follow and adhere to all the COVID-19 Policies and Protocols

Additional Requirements:

  • Experience providing support to people with a disability and their families is essential
  • Excellent problem solving and crisis management skills in a variety of challenging social situations is an asset
  • Ability to adapt well to constantly changing work environment and expectations
  • Demonstrated experiences in supporting people in a variety of different situations both at home and within the community will be an expectation
  • Experience supporting people with a variety of higher degree medical support needs
  • Experience supporting and understanding the individualized needs of people who do not use words to communicate and creating/implementing augmentative communication tools
  • Experience supporting people with disabilities with full participation, inclusion, and citizenship in the community
  • Demonstrated understanding of supporting a planning process and a demonstrated ability to assist people to achieve their goals will be an expectation
  • Demonstrated past experience supporting people living with mental health supports
  • Ability to effectively communicate with people both internal and external to the organization and to professionally represent CLPE
  • Excellent interpersonal skills
  • Ability to work both independently and as part of a team
  • Demonstrated past experience in taking a leadership role with the team to initiate tasks combined with creative problem-solving skills
  • Knowledge of community resources and an understanding of supporting people holistically
  • Demonstrated experience with administrative tasks
  • Experience with facilitating and participating in professional meetings would be an asset.
  • Understanding of Ministry compliance reviews and experience with maintaining expected documentation
  • Excellent oral & written communication skill
  • Past experience providing leadership to team members to ensure they understand and practice organizational values and to ensure delegated tasks are completed within the expected time

Position Requirements

The coordinator of the Community home provides support to enhance the quality of life for people with intellectual disabilities in living in a community-based home; the day-to-day lifestyle will be directed by the person and will be based on the goals the person has for their life. The coordinator will provide leadership in the community on behalf of people to ensure access to leisure, recreation and social opportunities and will work effectively as a team member, supporting the team and promoting the Mission and Vision statements of CLPE.

Job Duties Include:

  • Supporting people with disabilities with full participation, inclusion, and citizenship in the community
  • Providing supports and services that are designed for people as determined by their needs and wishes.
  • Supporting and promoting that people are treated with dignity and respect, both within their home and their community
  • Supporting people to have an active and meaningful role in the social life of their community.
  • Supporting people to advocate for their rights and freedoms.
  • Assisting people with the making meaningful connections within the community and with their friends and families
  • Facilitating and taking a leadership role with planning with people receiving support
  • Supporting people to have opportunities to grow, become successful and be fulfilled.
  • Assisting with the day-to-day activities in both a residential and community services setting inclusive of meal preparation, personal care and daily management of the home.

Hourly Wage/Salary: To be determined

Hours: 80 hours every 2 weeks - shift is relative to individualized needs/job requirements (includes a variety of day, evening, & weekends shifts)
