Youth Habilitation Quinte Inc. requires a Transitional Connector 30 to 35 hours per week who will promote independence and quality of life through the coordination of required support services for young people 16 – 24 years of age.  The office will be based in Belleville and the position will service Hastings and Prince Edward Counties.

The successful candidate will oversee the transition of the young person from the children’s mental health system to the adult mental health systems and ensure the appropriate supports are in place to facilitate the completion of the transition process.

• The candidate must possess excellent communication, organization and facilitation skills and have a strong working knowledge of the children’s and adult mental health systems in the Hastings County area.
